If you have arrived at this page it is because you are serious about taking care of your Whole-Body and/or have exhausted outside resources and/or you want to follow the Holistic health lifestyle.

At The Gainesville Holistic Health Center, healing is based on a partnership and a process. To find and treat the root causes of your illness, you will have to work hard and so will we.
​
It is important for us to know as much as we can about you in order to take the best approach and to treat the Whole-“U”.
That said, if you have any blood results (averaging for the past 3-6 years), imaging (X-Ray, MRI, CT scan or others), other genetic testing or others type of tests/diagnosis/results we would greatly appreciate it if you would send them to us 3-7 days prior to your appointment so we can review the information and have a better understanding of your current or past situation. We want to spend our time wisely with you during your first encounter with our practitioner(s).
We also strongly recommend a free consultation with Dr. Stéphane Provencher, owner and brain behind the whole-listic approach, to make sure your needs and goals will be met at our center. All of our other practitioners also offer a free 20 minute consultation for YOU to see if our vision matches yours – because YOU are important!

After the Free Consultation, then ?
Step 1 - INTAKE FORM: When you have received your PIN via email (info@ghhcenter.com) please follow the email instructions or click HERE to complete our intake forms. (P.S. You will need to create a new PIN; please remember it since it will allow you to sign in during your first visit)
STEP 2 - INSURANCE (Chiropractic services ONLY): If you have insurance (click HERE to find which insurance plans we participate in), please download these forms and fill them out prior to your visit. These are questionnaires that tell your insurance company the seriousness of your complaint. These forms are also requested if you schedule an appointment and it has been more than 30 days since your last visit. (FORMS)

STEP 3 - YOUR FOOD HABITS: Please fill out these forms (N1, N2) 3-7 days prior to your appointment.

STEP 4 - MEDICAL RECORDS: Please collect your pertinent or recent lab tests, radiology reports, or other medical reports which you feel may be helpful to evaluate your overall health. You may request these medical records from your family physician, any specialists, and/or hospitals (only the past 2 years).

Scan and email all your records or fax (571-248-0964) them to our office prior to your first visit. Please call us if you need any help with this.

Also, please write down all medications and supplements you are currently taking and send them prior to your visit: email or fax.

STEP 5 - Emotions:
Please list emotional traumas or hurts you have encountered in your life. If you are facing a life-changing decision or changing directions, please let us know. Also include when you had your children, marriage, divorce, break-up, move, new job, etc...

Car Accident or Work Related Accident (Work-Comp) (Chiropractic ONLY)
If you have been involved in a Car Accident, you will have to follow all the steps above and fill out these other Car Accident Specific forms, click on them to print and fill out (PI1, PI2).

If you have been involved in a Work related accident, please consult your WorkComp agent prior your first visit and and see if we can be of service. WorkComp companies dictate your care and some have their own protocols.
What to Bring at your First Visit ?
Please bring your driver’s license, insurance cards and email all paper filed under FORMS, N1, N2, all imaging and medical records PRIOR your appointment.
If you were involved in a car accident (chiropractic services only): all the above plus all car accident details (PI1, PI2).
We encourage you to bring any nutritional supplements that you take regularly including any vitamins, minerals or herbal supplements. Our practitioners will want to verify & test the ingredients and strengths of each supplement to get a better idea about the nutrients you are receiving. (applicable with our nutritionists or naturopath)

Oh! Remember your new PIN since we will have you sign in upon your arrival. It is easier and also saves paper. Furthermore, you have the opportunity to tell the practitioners if your condition has changed or if a symptom is new. There is a space to add additional comments related to your health (emotion, mental, stress, physical or other).
Patient Doctor/Practitioner Relationship
Your consultation will only be as good as the information you are willing to share. In order to get the most out of your consultation, you must be open and honest about any symptoms you are experiencing, medications you are taking, health history and lifestyle choices. This information helps our doctors provide you with the best care you need. Please be assured that all information you provide is confidential.

Your First Visit
​
For your initial visit with your integrative medicine physician or practitioner, plan on spending 60-90 minutes.
During the initial intake, your doctor/practitioner will review all of your medical records and discuss any previous diagnoses, treatments, and medications you currently take. All of the information on the intake forms will be discussed in great detail and diagnostic testing may be ordered to help identify the root cause of your condition. Then, our team will meet and develop an individualized treatment plan based on the findings using a functional medicine approach.

A significant portion of your visit will consist of patient education. Your practitioner will address and answer all of your health questions. At Gainesville Holistic Health Center, we believe it is important for you to understand your body and your health. We spend time making sure you understand your medical conditions and the importance of finding the root cause of the condition.

Labs
At GHHC we do not have a lab for testing, however our integrative medicine physician might order conventional labs, or state-of-the-art functional medicine testing. Please read below about the different types of lab testing we offer.

Conventional Lab Testing
​
Conventional labs are those that you are already familiar with. These are the labs such as thyroid testing, vitamin D and cholesterol, to name a few. There are two ways of paying for conventional lab testing:

Conventional Lab Testing Option 1: Submit through your Insurance
We have a partnership with LabCorp or the Heath Cote Hospital. If you live in Gainesville VA. and have insurance coverage, we recommend allowing to either lab to submit these tests through your insurance. This means that GHHC will not collect payment for these tests. We recommend calling your insurance provider prior to your initial appointment.

Conventional Lab Testing Option 2: Prepay to GHHC
GHHC’s partnership with Professional CO-OP, a LabCorp partner, allowing us to get labs at a discounted rate and we pass this discount on to you. If you have minimal insurance coverage, a high deductible or live out of state, we recommend this option. GHHC will collect payment for these labs and because of the highly discounted rate you may not file for reimbursement with your insurance provider.

Functional Medicine Lab Testing
Our practice uses high-quality, cutting-edge functional lab tests in order to get the most accurate information about how your body is working and to uncover the root cause of your illness. These tests may or may not be covered by your insurance company and therefore we have you pay our office for the labs and we pay the lab company directly on your behalf. You may also use funds from a Health Savings Account (HSA) to cover the costs of these tests.
